For your convenience, we've provided our patient forms below. To help expedite your first visit, please click the button to download, print, and complete the forms. Bring them with you when you come in for your first visit.
If you choose to fill out the patient forms at our office, please arrive 15–30 minutes before your appointment to allow enough time to complete the paperwork prior to your appointment.
Accepted Health Insurance
In-Network:
We are in-network for most major insurance companies, such as:
- Medicare/Medicare Advantage
Blue Cross, Blue Shield/Anthem
State Fund WC
VA/Triwest
We are happy to verify all insurance benefits prior to your first visit. Even if we are an out-of-network provider, it may only mean a slight increase in your deductible or copay.
No Insurance?
Alpine Physical Therapy offers a self-pay option. There is a $160.00 per visit initial evaluation fee, and all follow-up visits are $85.00 per visit on a self-pay basis.
